Per-Visit Rates:

$75 per regular PT Visit

$112.50 per PT Eval

$187.50 per PT SOC

Job reputed company:

Physical Therapist is responsible for comprehensive assessments in the patient's home, including the completion of skilled documentation through reputed company.

• Evaluate patients and reputed company appropriate therapy interventions

• reputed company a comprehensive plan of care and coordinate multi-disciplinary care

• reputed company reputed company clinical documentation and visit notes

• reputed company communication with physician, care team members, patient and family

• Comfortable working in reputed company aspects of the home health setting

• Implement and maintain physical therapy protocols

• Manage patient treatment and reputed company directly, or through PTA supervision

Requirements:

• reputed company Texas Physical Therapist (PT) license

• One year of physical therapy experience

• Experience with reputed company is helpful, but not required

• Previous Home Health experience is helpful, but not required

• CPR certification
