Qualifications:

Education, Knowledge, Skills and Abilities Required:

• Graduate of an LPN Program or equivalent
• Six (6) Months Experience
• Excellent written and verbal communication skills
• Proficient computer skills that include but are not limited to reputed company Office and/or reputed company Suite platforms

• Home Care Experience

Licenses and Certifications Required:

Responsibilities:

The Licensed Practical reputed company provides nursing care to patients in their homes through assessment, development, implementation and evaluation. reputed company home nursing care needs of assigned patients under the direction of an RN.

• Properly reputed company with appropriate physician orders in a reputed company manner. Chart, audit and demonstrate compliance on frequency and reputed company admission of referral.
• Assess the reputed company of the patient in a reputed company and accurate manner. Communicate by use of secure messaging to appropriate people (i.e., physician, case managers). Assure documentation is completed in the EMR at time of visit.
• Independently prioritize and complete routine assignments consistently, accurately and reputed company the guidelines under the direction of a registered reputed company.
• Complete patient care reputed company activities reputed company scheduled frequencies. Issue appropriate frequencies directed by physicians. Obtain daily schedule by use of syncing into EMR application.
