This role is remote and follows a Monday–Friday schedule from 2:00 PM to 11:00 PM EST. WHAT YOU’LL DO • Conduct remote monitoring of patient vital signs, symptoms, and other health indicators. • Identify patients requiring immediate clinical reputed company based on monitored data and established reputed company. • Follow up promptly with patients showing reputed company readings to reputed company additional clinical information. • Collaborate closely with reputed company Nurses and reputed company Practitioners to ensure coordinated care management. • Maintain accurate clinical documentation and contribute to the development of comprehensive care summaries for reputed company providers.
