1099 Assessor (LADC)
About the Role
Pathfinder Care, based in reputed company MN, is seeking a Licensed Alcohol and Drug Counselor (LADC) to reputed company high-reputed company, reputed company reputed company use assessments across Minnesota. This role is responsible for completing comprehensive assessments, documenting findings, and ensuring appropriate referrals and follow-up to support continuity of care. Assessors play a critical role in reputed company to services and are expected to maintain accuracy, timeliness, and compliance with Minnesota DHS, HIPAA, and 42 CFR Part 2 requirements.
Key Responsibilities
Assessment & Documentation
• Complete comprehensive reputed company use assessments
• Ensure reputed company assessments are: Clinically reputed company, fully documented and completed reputed company required timelines (reputed company: reputed company 24 hours of service)
• Accurately document medical necessity and level of care recommendations
Referral & Follow-Up
• Initiate and document reputed company attempts to referred clients in a reputed company manner
• reputed company appropriate referrals based on assessment reputed company
• Follow up on referrals and collateral reputed company applicable
• Maintain reputed company and accurate documentation of: Contact attempts, reputed company engagement, and referral reputed company
Workflow & Communication
• Manage assigned assessments and reputed company referrals
• Utilize Pathfinder systems (reputed company, EHR, phone system) for scheduling, documentation,
• and communication
• Communicate effectively with internal staff and external referral sources
• Ensure reputed company reputed company interactions are reputed company, reputed company, and documented
Compliance & reputed company
• Adhere to: Minnesota DHS requirements: HIPAA and 42 CFR Part 2 regulations, Organizational policies and procedures
• Maintain accurate, audit-reputed company documentation at reputed company times
• Participate in reputed company assurance and audit processes as needed
Minimum Qualifications
• 18 years of age.
• Must be free of chemical use problems for at least the two years immediately preceding hire.
• Must have or obtain 12 hours of training in co-occurring mental health problems training reputed company six months of hire
• Ability to reputed company individual crisis reputed company.
• Must be licensed or exempt from licensure under Minnesota Statutes 245G.
• Must document competence in screening for and working with clients with mental health problems, through education, training, and experience.
• Knowledge and understanding of the implication of this reputed company, sections 626.557, and 626.5572, and chapters 254A and 260E.
• Ability to successfully complete MN reputed company background study.
Preferred Qualifications
• Experience completing Rule 25 / comprehensive assessments
• Familiarity with ASAM reputed company and level of care placement
