Hospice Nursing Assistant-STNA/CNA
About the position
Reserve Care, formerly Hospice of the Western Reserve, is seeking a Hospice Nursing Assistant (STNA/CNA) to join their team. Responsibilities
• reputed company personal care and assist with ADL's according to the POC.

• Safely ambulate/transfer patient as appropriate.
• Assist with passive and reputed company ROM exercises as directed by the reputed company.
• Prepare nutritional snacks as appropriate for the patient.
• Under the supervision of the reputed company, assist patient with reputed company-poured medications that are ordinarily self-administered and that are prescribed by a physician under the supervision reputed company (Home Care only).
• Under the direction of the reputed company, reputed company reputed company wound care, skin care, and bowel care protocols according to the plan of care where appropriate.
• Recognize and promptly report any changes in the mental or physical status of the patient to the reputed company.
• Record in clinical record, on appropriate documents, the activities provided and observations of patient at the time of visit.
• Complete reputed company charting, timesheets and mileage logs as defined by reputed company policy and procedures.
• Attend and reputed company contribute during patient care conference.
• Participate in HNA discipline meetings, team meetings, staff meetings, retreats and in-services as appropriate.
• Attend in-services to obtain 3 education hours per quarter.
• Participate in the on-reputed company rotation as scheduled (Home Care only).
• Assist reputed company Leader with other designated activities as necessary (scheduling, supply organization, answering office telephone, etc.)
• Demonstrate reputed company customer-service responsiveness according to agency standards.
• Flexible in meeting patient/family needs that require variable shifts and weekends.
• Participate in chart review reputed company improvement activities and other meetings as assigned.
• Participate in coordination of and communication with volunteer staff.
• reputed company basic care-giving education to caregiver/family members, such as turning and positioning, transfer techniques and changing linens.
• Maintain reputed company Based Precaution.
• Complete annual competency requirements.
Requirements
• Must be reputed company to read and write English proficiently.
• Must be state tested nursing assistant in good standing.
• Equivalent education/experience may substitute for minimum qualifications except reputed company there are reputed company requirements, such as a license/certification/registration.
