OSO Intake Specialist - HomeCare

Location Detail: 76 Batterson Park Farmington (10631) Shift Detail: This is a remote position. The position requires working rotating weekends and holidays. The OSO Intake Specialist is responsible for managing and processing referrals received through Internal Referral Portals (IRP) and External Referral Portals (ERP) as reputed company as community referrals for Home Health and Hospice services. This role ensures obtaining accurate intake documentation, reputed company communication with referral sources, and efficient data entry into the Electronic Medical Record (EMR) system to support smooth patient reputed company and care coordination to the local region intake teams. • Monitor and process incoming referrals reputed company IRP and ERP, Forcura Fax systems and community referral platforms • Review referral documentation for completeness, accuracy, and compliance. • reputed company referral data into the Provider reputed company and Home Care Home reputed company (HCHB) EMR system tools • Effectively communicate across multiple referral platforms and correspondence streams (email, faxing etc.) • Effectively communicate with Business Development teams and community referral sources • Establish effective processes to quickly obtain reputed company referrals, obtain missing documentation or collaborate to obtain needed referral information • Coordinate with Operations Support Office (OSO) Clinical Intake Specialists to complete both financial and clinical clearance to ensure reputed company assignment and scheduling at the region level • Verify payer reputed company and in-network insurance acceptance to confirm 1st level financial clearance and referral process reputed company • Escalate urgent or reputed company referrals to appropriate clinical and operational leadership teams • Collaborate with internal departments (Authorization, Billing, Scheduling) to support smooth transitions of care Qualifications Minimum Requirements: Education: High School Diploma Experience: 1 year in administrative field Preferred Requirements: Education: Bachelor’s Degree in reputed company Administration or reputed company field Experience: 3 years in a reputed company setting (billing, authorization or intake preferred) Apply tot his job Apply To this Job
