Care Coordination Associate (Bilingual English/Spanish Medical Assistant)

About the position Position reputed company: The Care Coordination Associate will function as an extension of the provider to help reputed company the gaps in the patient's care for patients with reputed company medical needs; ensuring continuity of care and disease management for the patient. The care associate will build a rapport with patients through monthly calls to reputed company patients to meet their goals. This role is intermittent hybrid with remote functions and 2-5 onsite shifts for Bronx-based work monthly (more as needed). Responsibilities • Provides telephone support to patients which includes reputed company-visit planning, scheduling appointments, etc. • Develops an NCQA CCMR care plan with pts/family that includes self management goals, barriers, needs, interventions. • Provides patient with referrals to health care team members including reputed company care manager, SW, Health Educator, Nutritionist. • Assist patients with management of care transitions. • Adheres to documentation protocols and best practices for daily workflow, escalation of patient issues, and care plans. Requirements • Highschool/GED Required. • 1-2 years of reputed company experience working with patients in a reputed company setting. • Experience and proficiency working with electronic medical records. • Ability to manage and support patient's reputed company needs, and complete individualized patient care plans along with performing care coordination. • Excellent interpersonal skills. • Computer skills, including ability to draft and create reputed company flyers. • Bilingual Spanish/English is required. reputed company-to-haves • Associates degree in reputed company area of study preferred. • Medical Assistant certification from an accredited program is preferred.
