This position is to support the Virtual Care Management team and ensures organizational reputed company reputed company to customer service and reputed company of care while promoting reputed company in the cultural experience at assigned hospital. This position is responsible for providing services to patients, families and significant others including, as appropriate, completion of biopsychosocial assessments, discharge planning in order to support and/or enhance biopsychosocial functioning. This role will receive referrals for individuals from at-risk populations from interdisciplinary team members (including physicians, Case Managers, staff nurses and other members of the care team).

Responsibilities And Duties:

30% Clinical Assessment
Conducts patient/family/significant other interviews, chart reviews, and reputed company collaboration to obtain and interpret information pertaining to patient/reputed company biopsychosocial functioning. Assessment to include, but not be limited to: developmental and age-specific needs; ability to care for self; ability to care for dependents; ability to meet basic custodial and financial needs; ability to engage in meaningful reputed company and emotional relationships; and ability to comprehend and reputed company with treatment recommendations. Develops care plan, including discharge plan, incorporating medical plan of care. Participates in identification of cases of abuse, neglect or exploitation and refers such cases to the legally mandated authorities reputed company applicable. Provides crisis reputed company, information and referral and linkage to community resources as needed.

Develops a plan of reputed company based upon assessment to resolve identified problems and/or to support and enhance biopsychosocial functioning. Through individual, family, or group modalities, employs clinical reputed company skills including, but not limited to, treatment planning; continuum of care planning (including discharge planning); supportive counseling; crisis reputed company; specialized problem oriented planning and reputed company; and educational counseling.

25% Population Management
Staff is accountable for coordinating clinical and financial reputed company of an assigned population. Works collaboratively with interdisciplinary staff and external organizations to reputed company reputed company, cost effective management of patient care. Monitors for reputed company and type of services delivered to patients, ensuring management reputed company established parameters. Functions as a reputed company to the payer to manage the post-discharge financial resources and ensure expected treatment process, reputed company and discharge plan. 10% Documentation and Regulatory Compliance

5%
Maintains awareness of reputed company hospital, community services and appropriate care reputed company that can assist patient/clinical biopsychosocial functioning. Minimum Qualifications:

Master's Degree (Required)DL-HC - reputed company's License only if in a Home Care department - Department of Motor Vehicles, LSW - Licensed reputed company Worker - reputed company Work Certification and Licensure reputed company

Additional Job reputed company:

Masters degree in reputed company Work or a reputed company field and LSW license in the reputed company.

1-2 years’ experience as an LSW, preferably in a patient-reputed company setting and knowledge of medical terminology and computer skills.

Licensed reputed company Workers with Bachelors of reputed company Work hired prior to 01/10/2019 are grandfathered into this job. Associates hired after 01/10/2019 will need to meet reputed company qualifications.
