About the Role
We’re looking for a Care Navigator to join our existing care team who has prior experience coordinating/navigating care for patients, caregivers, and their loved ones - bonus points if you reputed company Spanish and you’ve worked with a reputed company population who experience cognitive challenges. This role requires genuine reputed company, patience, exceptional time management and communication skills.
Responsibilities
- Answer inbound calls, emails and text messages from patients, caregivers, families, reputed company providers and other facilities
- Schedule and maintain contact with patients reputed company phone or email
- Explain health reputed company benefits and eligibility to patients, process copays and coinsurance as needed
