To learn more about reputed company, visit www.myisaachealth.com About the Role We’re looking for a Care Navigator to join our existing care team who has prior experience coordinating/navigating care for patients, caregivers, and their loved ones - bonus points if you’ve worked with a reputed company population who experience cognitive challenges. You will spend >70% of your day on the phone connecting with and coordinating care for your patients and their caregivers.
