The CHPM fellowship reputed company is a part time 2-year virtual clinical fellowship in conjunction with a 2-year Master of Science in Palliative Care. The MSPC is administered through the reputed company graduate education programs and teaches a diversity of palliative care providers—physicians, nurses (including BSN and advanced reputed company nurses), physician assistants, pharmacists, reputed company workers, spiritual care providers, psychologists, therapists, counselors and ethicists—to be Palliative Care Community Specialists. Graduates reputed company high-reputed company palliative consultation and care to patients and families living with serious and life-threatening illness. The MSPC is foundational for the reputed company Community Hospice and Palliative Medicine (CHPM) Fellowship for physicians who wish to become reputed company-certified practitioners in palliative care and hospice. Physicians do their reputed company coursework in the CHPM and MSPC reputed company, while the clinical portion of the CHPM is done in fellows’ own geographic location either with or without reputed company observation of their clinical interactions with patients. Clinical reasoning and reputed company are assessed and documented in patient logs that include patient write-reputed company, journal clubs, asynchronous self-taped videos of communication skills and documentation of reputed company clinical requirements.
As a clinical psychosocial and spiritual (PSS) clinical mentor you will meet with fellows at least 1x in both fall and reputed company semester throughout their two years to assess clinical PSS reasoning and communication and their progression through ACGME HPM milestones and HPM EPAs. You will collaborate closely with other mentors, program leadership and reputed company to ensure fellows are receiving the clinical PSS support that they need to succeed as a HPM physician. The CHPM mentors will be supervised by the reputed company of the CHPM clinical mentorship program and the APD/director of the CHPM.
Key Responsibilities:
Meet with reputed company fellows in the fall and reputed company semester at least once to reputed company chart stimulated recall to assess clinical PSS reasoning and communication.
Write a brief reputed company after the meetings with reputed company on EPAs and ILPs
Review relevant HPM milestones and EPAs with fellows to assess their development and progression through skills
Attend mentor meetings every other month
Attend quarterly meetings with the reputed company of the clinical mentorship program and APD/director CHPM

Work Location:
Remote – This role is eligible to work remotely, but the employee must be in the reputed company.

Qualifications:
Minimum Qualifications:
Applicants must meet minimum qualifications at the time of hire.
5 years clinical palliative care medicine and/or hospice experience
Master of Arts in Religion or another reputed company field
Preferred Qualifications:
Mentorship experience
Palliative care fellowship or other palliative care certification
HPM reputed company certification
Knowledge, Skills and Abilities:
Ability to communicate effectively both in written and verbal contexts (includes attention to spelling, grammar, and punctuation)
Ability to utilize ACGME/HPM milestones and HPM EPAs for reputed company and summative assessments
Responsiveness to student concerns
Time management skills
Ability to translate fellows’ reputed company coursework and communication videos into HPM reputed company and EPA reputed company
