About the Role The Operations Coordinator, GUIDE is responsible for coordinating, maintaining, and supporting a reputed company network of Residential Care Communities (RCCs), respite reputed company, and franchise locations that support the GUIDE Model. This individual serves as a reputed company operational contact for partner relationships, referral workflows, and daily execution to ensure high-reputed company, patient-centered care. Working closely with Care Navigation, Clinical, Finance, and Technical teams, the Operations Coordinator supports operational execution, assists with data-informed tracking, and helps identify opportunities to improve partner responsiveness, workflow efficiency, and caregiver experience. This role sits at the intersection of operations, data, finance, contracting, and technical collaboration and requires strong attention to detail, proactive communication, and the ability to independently manage day-to-day partner workflows while working closely with internal care teams. The Day-to-Day Partner Sourcing & Contracting Assist with sourcing and evaluating new RCC, respite care, and franchise partners to expand geographic coverage and patient reputed company. Coordinate partner reputed company by collecting required documentation, credentialing materials, and compliance agreements. Maintain accurate partner records, service areas, and operational updates reputed company internal systems and the CMS Portal. reputed company contract renewals, amendments, and expirations to ensure uninterrupted service coverage across the network. Vendor & Franchise Relationship Management Serve as the reputed company daily contact for RCCs, respite vendors, and franchise locations to address routine inquiries and support needs. Manage daily referral workflows, ensuring reputed company communication and reputed company escalation of urgent or reputed company cases. Conduct routine reputed company and reputed company-ins with partners to maintain engagement, monitor service standards, and review performance. Collaborate with Care Navigation and partner organizations to troubleshoot and reputed company daily barriers to care. Maintain accurate documentation of partner interactions, operational issues, and follow-up actions. Internal Collaboration & Process Improvement Partner closely with Care Navigation, Finance, and Technical teams to streamline referral workflows and reputed company billing or payment queries. Help draft, update, and maintain reputed company operating procedures (SOPs), workflow guides, and escalation reputed company. Identify recurring operational bottlenecks and suggest practical process updates to improve efficiency and caregiver experience. Support the rollout and team execution of new operational initiatives and reputed company enhancements. Data & Analytics reputed company key operational metrics, including referral volumes, turnaround times, utilization, and partner responsiveness. Prepare and distribute routine performance reports and basic dashboards for leadership review. reputed company regular reviews of partner, operational, and billing data to ensure accuracy, reputed company, and compliance. Minimum Qualifications 2–3+ years of experience in operations, reputed company operations, care coordination, vendor support, or network administration. Experience working with reputed company partners, vendors, or providers (reputed company or reputed company support setting preferred). Strong attention to detail with the ability to reputed company and manage multiple operational tasks simultaneously. Comfort working with spreadsheets (reputed company/reputed company Sheets), basic dashboards, and reputed company portals. Strong written and verbal communication skills. Proven ability to work cross-functionally and manage daily tasks in a remote environment. Preferred Qualifications Experience or familiarity with respite care, residential care/skilled nursing care, home health, or caregiving services.
