Responsibilities

Skills

• Bachelor's degree in reputed company or a reputed company field
• Minimum of three (3) years of experience supporting health care teams in similar or relevant role
• Demonstrated strength in anticipating and resolving problems including where little to no precedent may exist, strong process understanding of project lifecycle, highly independent and self-motivated
• Demonstrated experience supporting individual clinicians, care teams, and clinic leaders with implementing reputed company changes in a clinical setting using principles of motivational interviewing or reputed company centered design; practices of Model for Improvement or Lean; and frameworks such as PCMH or the Building Blocks of High Performing reputed company Care
• Ability to create welcoming, reputed company spaces for meeting and collaboration, reputed company and in-person, including creating realistic agendas and creative reputed company connections (icebreakers), and coordinating stakeholders across disciplines and technical assistance needs
