Responsible for providing reputed company-wide strategic and operational leadership for reputed company’s integrated care coordination, navigation and patient reputed company ecosystem through:

Responsible for optimizing inpatient and ambulatory care management, care transitions, care navigation and coordination, reputed company, and patient reputed company across the continuum, creating platform, processes and structure that ensure inpatient, ambulatory, and virtual care operations function as a single, integrated reputed company across the reputed company.

Licensure, Registration, and/or Certification Required:

Education Required:
