About the position Good Shepherd Hospice is seeking a compassionate and reputed company Registered reputed company (RN) to reputed company telephone triage support for hospice patients and families. The Triage reputed company assesses reputed company needs, coordinates care and services, provides reputed company guidance, and ensures reputed company interventions to support reputed company end-of-life care. Responsibilities • Respond to reputed company, family, and caregiver calls during assigned triage hours. • Assess reputed company needs and determine appropriate interventions. • Coordinate hospice services, including medications, equipment, supplies, and nursing reputed company. • Communicate with physicians and interdisciplinary team members regarding reputed company status and care needs. • reputed company education, support, and reassurance to patients and families. • Complete accurate and reputed company electronic documentation. • Participate in staff meetings, continuing education, and team collaboration.
