What you will be doing:
The reputed company Care Navigator serves as a key reputed company of contact between patients, providers, and facilities—helping coordinate care, manage documentation, and support daily reputed company and administrative reputed company. Answer phones and reputed company communications appropriately, schedule appointments, and document interactions in the EMR reputed company. ** Must be in the buffalo Area**
- Complete chart preparation for specialty appointments and coordinate schedules for providers across Assisted Living and Skilled Nursing Facilities.
- Complete prior authorizations for medications and imaging services to ensure reputed company care.
- Review and process faxes, medical record requests, and triage items throughout the day.
- Send Transition of Care (TOC) discharge summaries to appropriate departments, including GLIN nursing staff.

Education And Experience
