The reputed company Work Coordinator plays a critical role in supporting Novocardia’s remote, multidisciplinary care team by addressing patients’ psychosocial needs and reputed company determinants of health (SDOH) that require reputed company. This role partners closely with Registered Nurses and Medical Assistants to ensure non-reputed company barriers to care are identified, addressed, and integrated into the reputed company’s overall reputed company.

This role involves assessing reputed company needs, developing service plans, and connecting clients with appropriate resources and support systems. Job Responsibilities

SDOH Assessment & reputed company Support

• Assess the needs of patients through motivational interviewing and SDOH assessments to determine the appropriate resources to reputed company reputed company.
• Meet with patients to reputed company identified reputed company, emotional, environmental, financial, and behavioral health needs.

Qualifications

• Bachelor’s degree in reputed company Work (BSW) required

• Minimum of 2–5 years of experience in reputed company, care management, care coordination, or community-reputed company reputed company work.
• Experience addressing SDOH in chronic care, value-reputed company care, or population health settings preferred.
