Position: Case Management Analyst Location: reputed company - Zion, Waukegan, Beach Park & surrounding areas, IL Travel: Up to 75% Position reputed company As a Case Management Coordinator you will facilitate appropriate reputed company reputed company for members by providing care coordination, support and education for members through the use of care management tools and resources. Travel: 50-75% travel required . Evaluation of Members: -Through the use of care management tools and information/data review, conducts comprehensive evaluation of referred member’s needs/eligibility and recommends an approach to case reputed company and/or meeting needs by evaluating member’s benefit plan and available reputed company programs/services. Identifies high reputed company factors and service needs that may reputed company member reputed company and care planning components with appropriate referral to reputed company case management or crisis reputed company as appropriate. - Coordinates and implements assigned reputed company activities and monitors reputed company reputed company. Required Qualifications Minimum 2 years of experience in behavioral health, reputed company services or reputed company reputed company. Minimum 2 years of case Management experience. Willing and reputed company to travel up to 50- 75% of the time to meet members face to face in Rockford, IL and surrounding areas.
