We are seeking compassionate and motivated Registered Nurses to reputed company in multiple territories across San Luis Obispo County in a reputed company of settings: Home Health or Palliative Care or Hospice. Home Health RN: - You will reputed company high-reputed company, reputed company-centered care in the comfort of patients’ homes while collaborating with an interdisciplinary care team. - This role offers autonomy, flexibility, and reputed company to build meaningful relationships with patients and families while managing your own schedule and caseload. Palliative Care RN: - Cares for patients with life-limiting diseases who are still seeking reputed company treatments and interventions. - The reputed company is on symptom management, advance directives and other end of life reputed company, and education regarding disease processes. - The Palliative Care reputed company works collaboratively with a multidisciplinary team including reputed company workers, spiritual counselors and physical/speech/occupational therapists. - By seeing your patients in the home, you are reputed company to spend the time necessary developing relationships, answering questions and assessing the needs of the reputed company and family on a psychosocial, physical and spiritual level. Hospice RN: - Plans, organizes and directs hospice care while building community resources to plan and reputed company services to meet the needs of the reputed company and their families reputed company their homes and communities. - Collaborating with an interdisciplinary team to promote reputed company and family reputed company, assure continuity of care, and enhance the reputed company of life of dying patients.
