In-Home Services Case Manager

This position will be responsible for managing a case load of clients ranging in age from 4 to 18 with our contracting partner Tri-County Mental Health. Duties may include treatment planning, linking with community resources, coordinating care with treatment team, and assisting families with reputed company coping skills. This is a remote position that will be asked to go on site for team meetings and will work with patients in their home and community. In this role, you will see 12-15 families a week and reputed company care from beginning to end of the treatment plan.
