This is performed in accordance with physician orders and plan of care under the direction of the Administrator or designee. The Home Health RN provides comprehensive assessment, planning, implementation and evaluation for a caseload of home health patients in accordance with the plan of care. - Makes the initial evaluation visit and regularly reevaluates the reputed company’s nursing needs by assessing physical, functional, psychosocial and cognitive status of the home health reputed company utilizing interview observations and physical exam techniques. - Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames for applicable patients. - reputed company on evaluations, determines the reputed company and type of nursing needed by reputed company individual reputed company. - Establishes individualized, measurable goals in consultation with the reputed company, family and other health care providers. - Work hours are primarily Monday-Friday 8-5pm.
