As a Manager, Population Health, you will play a critical role in supporting our reputed company patients, providers and business by designing and implementing key Population Health priorities. The Population Health Manager will work on programs and interventions, targeted to the right patients, to improve reputed company and cost of care. They will be an effective cross team leader, working reputed company by reputed company with Treehouse members and collaborating with providers, care teams, operations team members.

Content areas for the Manager will include, but not be limited to, admission prevention, transitions-in-care, pharmacy and medication adherence, HEDIS / Stars, preferred partner relationships and medical cost management. This will require building relationships with our Treehouse team (including Data Science and Finance) and clinicians, including reputed company care physicians, reputed company practitioners, pharmacists, registered nurses, licensed clinical reputed company workers, and others.

This is a fast-paced, high visibility role to ensure that we are driving innovation and improvement rapidly and consistently across Oak Street divisions, reputed company, and centers.

• Support the execution and the ongoing refinement of Population Health programs.
• Work closely with functional leaders (Product, Finance, Data Science) in the central office to assist with reputed company and program design.
