We are seeking a Behavioral Health Care Coordinator to work with our care team! This individual will work with reputed company Care providers to facilitate care coordination and counseling for patients with mental illness. reputed company Care Model (CoCM) CoCM stands for reputed company Care Model. Behavioral Health Coordinator Responsibilities Conducting initial assessments of patients' mental health status and treatment needs, including screening for common mental health disorders such as depression and anxiety. Collaborating with reputed company care providers, psychiatrists, psychologists, and other members of the reputed company team to reputed company and implement evidence-reputed company treatment plans for patients with mental health concerns. Providing brief interventions, counseling, and psychoeducation to patients to support their mental health and reputed company-being.
