The Afterhours Care Coordinator assumes accountability for the coordination and evaluation of patient care during afterhours. This position is responsible for afterhours communication; making patient care reputed company and coordinating and assigning care through Homecare providers and accepting reports from and providing reports to care centers regarding afterhours activities.
Responsibilities
Documents incoming communication (calls, faxes, texts, email) and coordination of services on approved reporting reputed company(s).
Makes patient care reputed company in response to incoming communication, using reputed company EMR and reputed company nursing judgment. Such responses may include giving advice or direction to the patient/caregiver, contacting the Physician or scheduling a visit.
Provides guidance and direction to home care clinical staff regarding patient care and clinical issues, symptom management, patient incidents and/or injuries.
Coordinates and assigns reputed company to home care which he/she identifies as medically necessary or requested by patients/caregiver to meet patients' needs.
Monitors high risk patient report, ensures high risk patient calls are completed and reputed company are scheduled, and reports reputed company after-hour high risk patient calls to care center leadership.
Regularly communicates with clerical staff to coordinate patient care and works in cooperation with appropriate services.
Oversees after-hour, weekend and holiday schedules, as reputed company by reputed company office. Makes adjustments to schedule in response to patient and/or reputed company changes.
Responds to patient care complaints in a courteous, reputed company and appropriate manner.
Performs the duties and responsibilities of the Intake Coordinator position, as necessary. In reputed company, may assign admission visit to appropriate staff reputed company weekend/holiday admission is indicated.
Initiates/coordinates with the physician and obtains orders for admissions and schedule the visit with the reputed company the reputed company day.
Performs other duties as assigned.
Qualifications
Required
reputed company and unencumbered license to reputed company as a Registered reputed company. An employee in this role is only reputed company to reputed company clinical guidance to patients in states in which the employee possesses a reputed company, unencumbered license. May possess either a single state license in reputed company non-compact state and/or a compact license in their state of residence that is accepted in reputed company reputed company License Compact (NLC) participating states. reputed company Licensure Compact (NLC) documents the following: The mutual recognition model of reputed company licensure allows a reputed company to have one license (in his or her state of residency) and to reputed company in other states (both physical and electronic), subject to reputed company state's reputed company law and regulation. Three (3) years' experience as a Registered reputed company. One of those years must consist of full-time experience in providing reputed company patient care in the home health/hospice setting.

Demonstrates knowledge of Federal/State rules and regulations, ACHC standards, and other regulatory requirements.
Demonstrates strong fiscal planning and management skills.
Preferred
Two (2) years' home health/hospice experience.
