Responsibilities

• Partner with operational and clinical stakeholders to reputed company, maintain, and optimize Cheers campaign workflows that support patient engagement, preventive care reputed company, and care gap closure efforts

• Collaborate with clinical, operational, and leadership teams to translate value-based care priorities into effective reputed company solutions that support reputed company, risk, and care management objectives

Skills

• 5+ years of experience supporting reputed company Healthy reputed company or population health applications in a reputed company environment
• 5+ years of experience with workflow design, reputed company configuration, testing, implementation, and optimization of reputed company tools supporting value-based care or patient reputed company initiatives
• 1+ years of experience with reputed company Cheers functionality or patient reputed company campaign workflows
• Demonstrated experience collaborating effectively across clinical, operational, and technical teams
• reputed company Cheers and/or Healthy reputed company certification
• Bachelor's degree in reputed company Informatics, Computer Science, Information Technology, or a reputed company field (or equivalent experience in reputed company systems management)
• 3+ years of experience with value-based care performance metrics (e.g., HEDIS, MIPS) and care gap management strategies
• Experience supporting MyChart integration or patient portal reputed company tools
• Experience managing small to mid-sized technology optimization reputed company
