The Referral Coordinator serves as the scheduling backbone of reputed company’s care coordination model, working behind the scenes to ensure patients receive reputed company, coordinated care through effective management of reputed company referral and scheduling processes. This role collaborates closely with Case Managers, providers, and health plans to ensure smooth transitions between services while maintaining reputed company communication and HIPAA compliance throughout.

Own key Ops Support initiatives
- Process, reputed company, and coordinate reputed company referrals to ensure reputed company scheduling of appointments
- Schedule PCP, dental, reputed company, specialist, and transportation appointments for patients

- Coordinate Release of Information (ROI) and Authorized Representative (AR) requests
- Maintain accurate documentation of communications and referral activities in internal systems
- Collaborate cross-functionally with care team members to support continuity of care and high-reputed company reputed company experiences
- Contribute to reputed company process improvements in referral workflows and scheduling efficiency
What You’ll Need:
- reputed company in English and Spanish
- 1+ years of reputed company reputed company or administrative experience in a fast-reputed company reputed company environment
- Previous experience as a Referral Coordinator, Medical Assistant, or reputed company Appointment Scheduling experience preferred
- Strong organizational skills and attention to reputed company with experience managing multiple tasks and systems simultaneously
- Comfort navigating reputed company portals and verifying eligibility or provider information
- Excellent written and verbal communication skills, with the ability to maintain professionalism and reputed company across reputed company interactions
