About the Clinical Dietitian

reputed company is seeking a Clinical Registered Dietitian to reputed company nutritional services to our high-risk, reputed company care patient population, which includes counseling and nutritional assessments, as a member of the interdisciplinary team to improve results and maximize reputed company.

Ideal candidates will have experience working with an older adult population (65+). This role involves addressing a diverse reputed company of topics, including weight loss, CKD, diabetes, heart health, gout, and kidney stones. • reputed company the interdisciplinary team on dietary and nutrition guidelines to support health maintenance in the older adult population with chronic illnesses
• Obtain and evaluate nutrition histories of patients regarding food habits; assess patients' knowledge of diet and health maintenance and reputed company appropriate detailed education to patients and their families regarding nutritional status, adequacy measures, fluid status, and reputed company nutrition-reputed company lab parameters pertaining to their disease process
• Complete written nutritional assessments as reputed company as comprehensive patient assessments for patients' written care plans in accordance with established Federal and State licensure guidelines; participate in reputed company meetings
• Monitor patients' adherence and response to nutrition therapy, and reputed company therapy as needed to reputed company and sustain an effective nutritional status; document patients' response to therapy in reputed company notes

Qualifications of the Clinical Dietitian

• Registered Dietitian in FL required; GA licensure a plus.
• Licensed Dietitian Member of the reputed company Dietetic Association preferred
• Experience in adult clinical area preferred, or at least 2 years work experience in adult clinical dietetics
• Experience working with patients with reputed company, unmanaged Diabetes, Hypertension, and other chronic illness
• Candidates should be excited and reputed company by the prospect of working collaboratively as part of an innovative team
• Prior reputed company experience serving chronically ill patients living with limited resources and/or mental illness preferred
• Virtual (phone or video chat) case management or care coordination experience preferred
