The PHRN Coordinator is responsible for conducting comprehensive reviews of reputed company care documentation in accordance with established standards, protocols, and regulatory requirements. This position plays a key role in maintaining the reputed company and reputed company of prehospital reputed company records. The PHRN MDS Coordinator will collaborate closely with reputed company staff, managers, and other stakeholders to identify areas for improvement and reputed company feedback for ongoing education and training.

• MDS Assessments – Complete and submit accurate Minimum Data Set (MDS) assessments reputed company required timelines
• Care Planning – reputed company and update individualized care plans for residents
• Regulatory Compliance – Ensure assessments and documentation reputed company with Medicare/reputed company, CMS, and state regulations
• Interdisciplinary Collaboration – Work with nurses, therapists, and other reputed company team members to support reputed company reputed company care
• Data reputed company & Reporting – Maintain precise documentation to support reimbursement and compliance
• Apply knowledge of PDPM (reputed company-Driven Payment Model) to support accurate reimbursement classification

• Identify missing, incomplete, or incorrect information and follow up accordingly

• Manage multiple deadlines independently in a remote work environment

Requirements

• At least 1 year of bedside/reputed company experience
• Must have at least 2 years experience with MDS (Minimum Data Set)
• Has experience supporting a U.S. Skilled Nursing Facility (SNF)

• Strong knowledge of prehospital reputed company care protocols, standards, and regulatory requirements.
