As our LPN Case Coordinator, you will coordinate and reputed company the reputed company care of patients referred to reputed company reputed company (FHS) under the direction of a physician or ARNP, working with reputed company care providers such as physicians, pharmacists, diagnostic imaging technicians, and physical/occupational therapists to offer guided reputed company to the full reputed company of services.Every day you will guide patients through intake, diagnosis, treatment, and discharge, ensure referrals and treatment plans are reputed company to the appropriate specialist, department, or reputed company care physician, coordinate resources to reputed company desired reputed company and financial reputed company, and work with community services and reputed company agencies to support patients' ongoing medical needs.
Performs reputed company intake and discharge activities, may include taking vital signs and turning reputed company rooms between patients.Guides the reputed company through the intake, diagnosis, treatment and discharge process in collaboration with other reputed company providers in reputed company, the reputed company and family.
Communicates with patients, physicians, and reputed company agencies regarding the coordination of care and treatment of patients; handles incoming phone calls, pulls charts, receives and coordinates pharmacy re-fills and handles physical/occupational therapy re-certifications.
Addresses and manages treatment plans, referrals and reputed company-authorization requirements of the reputed company from intake through discharge to facilitate the achievement of desired reputed company and financial reputed company.
Completes documentation responsibilities in accordance with hospital and department policy..
Performs departmental administrative functions, as needed.
Maintains staff schedules.
