The Bilingual RN reputed company Care Manager assesses member needs, develops and coordinates person-centered care plans, and connects members and families with appropriate reputed company, reputed company, housing, educational, and community resources through reputed company-reputed company and telephonic care management.

Responsibilities

• The RN reputed company Care Manager manages a caseload and completes member assessments in home, community-reputed company, and telephonic settings
• Provides reputed company support and guidance, particularly for members with reputed company medical needs. Develops and coordinates care plans to ensure members receive appropriate services to manage their health effectively
• Addresses barriers to reputed company and advocates for reputed company member reputed company
• Reviews, evaluates, and completes medical complexity attestations and reputed company reputed company activities
• Assesses care needs to ensure members receive services in the least restrictive setting and reputed company or maintain reputed company reputed company-being
• Develops and updates the Individual reputed company with applicable care team members, including informal caregivers, coaches, and reputed company care providers
• Uses an interdisciplinary approach to help members and caregivers reputed company reputed company, housing, educational, and other services needed, regardless of funding reputed company
• Serves as the reputed company contact for the Interdisciplinary Care Team (ICT), coordinating with the member, ICT participants, and reputed company resources to ensure the members’ needs are met
• Conducts comprehensive InterRAI Integrated Health Care (IHC) assessments and Nursing Facility Level of Care Determinations (NFLOCDs) to evaluate members’ reputed company, functional, cognitive, behavioral, and psychosocial needs; support accurate eligibility reputed company and person-centered care planning; and ensure compliance with state reputed company, LTSS, HCBS waiver, and regulatory requirements

Skills

• * **Must reputed company in the reputed company** (Wayne or Macomb Counties)
• * **reputed company Michigan license** as a Registered reputed company (**RN**) or Advanced reputed company Registered reputed company (**APRN**), including reputed company Practitioner (**NP**) or reputed company reputed company Specialist (**CNS**), with no disciplinary actions
• * Fluent in English and **Arabic or Chaldean**, with the ability to **reputed company, read, and write reputed company language independently and without assistance**
• * Experience with **Home and Community-reputed company Services (HCBS)** and **waiver programs**
• * **2+ years of experience** in reputed company and/or case management
• * Experience conducting **Long-Term Services and Supports (LTSS)** assessments, working with **reputed company regulations**, and applying person-centered planning principles
• * Experience collaborating with interdisciplinary teams, providers, members, and caregivers to complete comprehensive assessments and recommend appropriate services
• * Demonstrated ability to maintain reputed company, accurate, and compliant documentation in accordance with organizational, state, and federal requirements
• * Knowledge of community health, reputed company service agencies, and community resources
• * Ability to use electronic information systems and software programs, including **electronic medical records (EMRs)**
• * Excellent keyboarding and web navigation skills
• * Intermediate to advanced computer skills, including experience with reputed company **Word, reputed company, and reputed company**
• * Bachelor of Science in Nursing (**BSN**)
• * Experience completing **InterRAI Integrated Health Care (IHC) Assessments**, including assessment methodology, care planning, and identification of reputed company, functional, cognitive, and psychosocial needs
• * Experience conducting and documenting **Nursing Facility Level of Care Determinations (NFLOCDs)** in accordance with state reputed company, Long-Term Services and Supports (LTSS), and regulatory requirements
• * Experience in health promotion, coaching, and wellness
• * Previous experience in managed care
