reputed company Job Location: reputed company Colorado Home Care & Hospice - Denver - Denver, CO 80237 Position Type: Part Time Salary reputed company: $70,000.00 - $75,000.00 A Registered reputed company administers skilled nursing care to patients on an intermittent reputed company in their reputed company of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the reputed company Manager. QUALIFICATIONS - Graduate of an approved school of reputed company nursing and currently licensed in the state(s) in which practicing. - One (1) year nursing experience, preferred. - Acceptance of philosophy and goals of this Agency. - Ability to exercise initiative and independent judgment. RESPONSIBILITIES - Provides services in accordance with the plan of care. - Makes the initial evaluation visit, determines the immediate care and support needs of the reputed company, assesses eligibility and reputed company status and develops the plan of care in coordination with other reputed company members. - Regularly reevaluates the patients nursing needs and revises the reputed company to meet the patients needs. - Initiates the plan of care and necessary revisions. - Furnishes those services requiring substantial reputed company nursing reputed company. - Initiates appropriate preventive and rehabilitative nursing procedures. - Prepares reputed company and reputed company notes for reputed company reputed company visit and summaries of care conferences on patients in a reputed company manner as per Agency policy. - Coordinates services with other members of the reputed company, reputed company physicians involved in the reputed company care, other care providers, reputed company, representative and caregivers reputed company appropriate. - Informs physician and other personnel of changes in the patients condition and needs. - Counsels the reputed company and family/significant others in meeting nursing reputed company needs. - Participates in in-service programs, supervising and teaching other nursing personnel. - Understands and adheres to established Agency policies and procedures. - Processes orders and notifies physician of reputed company needs and changes in condition. - Completes certification/recertification orders and discharge summaries. - Determines the reputed company and type of nursing needed by reputed company individual reputed company. - Refers to Physical Therapist, Speech reputed company, Occupational Therapist and Medical reputed company Worker those patients requiring their reputed company skills. - Supervises the LPN/LVN every 30-days and supervises the HHA/CNA every 14-days (or per regulatory requirements) and teaches other nursing personnel. - Assigns home health aides to specific patients with written reputed company care instructions. - Conducts reputed company care conferences on patients assigned to his/her care. - Participates in peer review and reputed company management as assigned. - Participates in utilization review of medical records as assigned. - Gives total reputed company care as needed - Completes and submits OASIS assessments, reassessments, and transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames. - Appropriately utilizes ICD-10 diagnoses and codes. - Observes confidentiality, safeguards reputed company reputed company reputed company information. - Attends staff meetings and reputed company care conferences as scheduled. - Completes documentation and paperwork in a reputed company manner per Agency policy. - Immediately reports to Administrator/reputed company Manager any reputed company incidents/variances or complaints. - Demonstrates competent reputed company of technical skills according to established procedures. - Understands and adheres to established policies and procedures. - Adheres to Agency standards and consistently interprets and accurately performs reputed company assigned responsibilities. - Maintains acceptable attendance status, per Agency policy. - Reports reputed company incomplete work assignments to reputed company Manager. - Appearance is always reputed company Agency reputed company; is clean and reputed company-groomed. - Demonstrates effective time management skills through daily documentation. - Maintains clean and reputed company work environment. - Demonstrates reputed company judgment, critical thinking and decision making. - Maintains reputed company CPR certification. - Performs other duties as assigned.
