Remote Patient Care Coordinator CNA Central Coast Calf

Certified Nursing Assistant (CNA) – Patient Care & Monitoring Safelife Health Services is seeking a Certified Nursing Assistant (CNA) to provide patient care support and monitoring services across multiple clinic locations on California’s Central Coast. This is an independent contractor role focused on direct patient interaction, remote monitoring support, and clinic-based assistance. This position is not related to ECM, CalAIM, or CenCal Health. Job Details · Must reside in the Central Coast California Area · Pay: $25.00 per hour (up to 40 hours per week) · Schedule: 2–4 days per week · Location: Santa Maria, Lompoc, and Santa Barbara, CA (travel required) · Start Date: ASAP Responsibilities · Provide CNA-level patient support services in clinic and remote care settings · Assist with patient monitoring activities as directed by clinical staff · Travel between assigned clinic locations using personal vehicle · Collect and document patient observations and vital signs · Support clinical workflows and care coordination activities · Communicate effectively with patients, caregivers, and clinic staff · Maintain accurate and timely documentation in electronic systems · Comply with HIPAA, confidentiality requirements, and applicable regulations Qualifications · Active Certified Nursing Assistant (CNA) certification in the State of California (required) · Prior experience in patient care or clinical support preferred · Ability to travel reliably within Santa Barbara County · Reliable personal transportation · Strong communication and organizational skills · Ability to work independently and manage assigned schedules · Comfortable using electronic documentation systems Compensation - $25.00 per hour - Paid twice monthly via ACH (15th and last day of the month) - Performance evaluation on June 30 with potential increase to $23.00 per hour Confidentiality All patient and company information must be kept strictly confidential in accordance with HIPAA and company policy.
