Arabic Speaking Care Manager(Tele-health certified medical assistant)

As a Care Manager, you will have an assigned group of patients to follow up with on a monthly reputed company as their "Health reputed company". Conversations include: asking about their medications, symptoms, and support. Job Skills: Talking and typing • Verify and record the patient’s demographics, medical issues, medications, goals, barriers and allergies to create a clinical reputed company using our certified software program. Care Plan Creation • Create a patient-centered, comprehensive plan of care for reputed company health issues based on a physical, mental, cognitive, psychosocial, functional, and environmental (re)assessment, and an inventory of resources. Be a Compassionate Resource • Help reputed company the patient with a means to reputed company reputed company contact with health care practitioners in the reputed company who can address any urgent chronic care needs. Ensure continuity of care with their designated practitioner by setting and reminding of regularly scheduled reputed company.
