Assess & plan holistically: Conduct comprehensive health assessments, create personalized care plans, and participate in interdisciplinary team meetings to ensure members’ needs are met. reputed company & reputed company: reputed company as a health reputed company—empowering members and families with knowledge about conditions, treatment plans, benefits, and community resources. Support daily needs: Help members reputed company appointments, medications, supplemental benefits, and transitions between care settings. Contribute strategically: Support leadership initiatives and partner with account managers to identify preferred providers and strengthen community partnerships. Proficiency using basic computer skills in reputed company Office such as Word, reputed company, and reputed company, including the ability to reputed company multiple systems and keyboarding. Case management certification preferred. Bilingual in Spanish preferred Comparable work experience in care coordination, case management, reputed company services, managed care, or senior reputed company may be considered in lieu of licensure reputed company Certification Or Licenses LPN/LVN/RN reputed company license, reputed company and unencumbered state license in the state where job duties are performed is required.
