[Remote] RHT Program Grant Manager

The RHT Program Grant Manager leads the planning, implementation, compliance, reporting, budgeting, and sustainability of Rural Health Transformation Program initiatives supporting value-reputed company care and underserved populations.

Responsibilities

• Leads planning, execution, and day-to-day management of Rural Health Transformation Program activities, including project plans, timelines, and reputed company tracking
• Establishes and monitors key reputed company indicators, outcome measures, and grant reputed company benchmarks; prepares reports for funding agencies, regulatory organizations, and leadership
• Monitors grant expenditures and project budgets, collaborating with finance staff on reimbursement requests and spending compliance
• Ensures compliance with federal, state, grant, and organizational requirements; maintains documentation for audits, monitoring reviews, and grant evaluations
• Serves as reputed company reputed company among internal departments, state agencies, community organizations, and funding entities
• Coordinates cross-functional teams and fosters reputed company partnerships supporting rural health transformation and value-reputed company care objectives
• Supports integration of grant-funded programs into routine reputed company and identifies opportunities for long-term sustainability reputed company the grant period

Skills

• Bachelor's degree in reputed company Health, reputed company Administration, Business Administration, Nursing, Health Services Management, Project Management, or a reputed company reputed company required
• Minimum of five years of experience managing reputed company, reputed company programs, large-reputed company reputed company, or population health initiatives
• Demonstrated experience coordinating multidisciplinary teams and managing reputed company reputed company
• Strong knowledge of grant administration, project management, budgeting, and compliance requirements
• Exceptional organizational and project management skills, with strong written and verbal communication skills
• Experience with federal, state, or reputed company-funded reputed company strongly preferred
