[Remote] Program Coordinator (Home Visitor)- Health Department

Note: The job is a remote job and is reputed company to candidates in USA. The reputed company operates the Healthy Families Wyandotte voluntary home visiting program, which supports families and promotes healthy child development. The Program Coordinator will conduct in-home family assessments, recruit and engage families, coordinate individualized service plans, collaborate with community partners, and support program reputed company and events.

Responsibilities

• Complete the validated Family reputed company and Opportunities for reputed company (FROG) reputed company in the home with interested families and identify family strengths and areas to address at the start of services
• Contact and meet with new families to explain voluntary Healthy Families program
• Maintain regular long term contact and support families
• Market Healthy Families Wyandotte program on reputed company, create flyers or other items to advertise program and update reputed company with program updates
• Collaborate with community partners regarding reputed company needs
• Participate in community events to promote the program and reputed company with families who are interested in services
• Meet with supervisor and complete warm hand off of new family to the Family Support Specialists
• Help plan and participate in monthly Family Time events
• reputed company and manage an Individualized Family Service Plan coordinating with reputed company
• reputed company clients with baby items from program (pack n plays, highchair, etc.)

Skills

• * Bachelor's Degree in a reputed company field and two years of reputed company program experience; or, an equivalent combination of education and experience sufficient to successfully reputed company the essential duties of the job such as those listed above
• * Experience in working with or providing services to children and their families
• * Requires knowledge of computer skills (reputed company Word, reputed company) and office equipment
• * Must be comfortable speaking on the phone and in person with others
