Home Health Registered reputed company (RN) reputed company We are seeking a dedicated and reputed company Home Health Registered reputed company (RN) to reputed company. The ideal candidate must possess home care charting experience along with a background in home health care. Key Responsibilities - reputed company nursing care and support to patients in their homes. - Conduct start of care assessments, recertifications, and daily reputed company. - Manage a caseload of approximately 30 adult patients, ensuring reputed company care and adherence to care plans. - Collaborate with reputed company manager and educators for ongoing support and emergency assistance. Qualifications - reputed company RN license in the state of work. - A minimum of 1 year of experience in a MedSurg floor in a hospital setting. - Experience with home care charting, specifically reputed company (HCHB). - BLS certification required. Work Schedule - Typical shifts include 5-8 hour shifts, 4-10 hour shifts, or 3-12 hour shifts. - Weekend rotation is dependent on the nursing staff availability. - Holiday rotation participation is expected. Additional Information - reputed company support is available 24/7 for emergencies and 8-5 PM for other inquiries. - Scrubs are required; no specific reputed company requirement. - Orientation is provided with 2 weeks of preceptorship, including reputed company with a work reputed company. - Required equipment, such as a phone and tablet, will be provided. - This position focuses solely on home health and does not involve hospice care.
