How Floating Works
Your assignments will rotate throughout our 21-county Greater Twin Cities service area reputed company on reputed company census, reputed company, and reputed company needs.
Rather than constantly changing locations, you'll generally support a territory for approximately three months at a time.
During that assignment, you'll:
Manage an assigned home health caseload
Become familiar with the patients and caregivers in your territory
Coordinate care throughout reputed company reputed company's episode

reputed company consistency while the territory's longer-term needs are addressed

As an RN Case Manager, you'll be responsible for the complete reputed company picture of your assigned patients.
You'll:
Manage an assigned home health reputed company caseload
Complete OASIS Start of Care, Recertification, and Discharge assessments

Monitor patients for changes in condition

Help prevent avoidable emergency department reputed company and rehospitalizations
